Equipment Glitch

My team is having a strange equipment glitch.

I unequipped level 1 chest armor from an old warrior then equipped it on Five right after purchasing him. It is now showing him using armor as his primary weapon! If that wasn't weird enough, a copy of the same armor was sitting in my vault. I equipped that and the one in his weapon slot didn't go away. But when I tried to unequip it, it STAYED equipped on him and now a 3rd copy of the armor is sitting in my only vault spot. I should only have 1 set of that armor not 3. Plus the one he is wearing seems to be making new ones when I try to remove it. Please help! I'm afraid to try do anything more with this warrior at the moment.
