I have a glad with max weapon skill but he still fumbles his weapon. Why is this happening? Does it have something to do with stats or fight strategy or something else?

A glad with maxed weapon skill can still fumble, it will just happen a lot less than a new glad with a low weapon skill.

Try lowering your activity level and blood lust and see if that helps. If you are set at 10 right now maybe see how you do at 8, and then watch it and adjust as needed.
